Building a form

Fields, validation, file uploads and what happens on submit.

Create and publish a form

  1. Open Forms and select New form. The button shows Creating form… while saving, then opens the new draft in its editor immediately. If creation fails, an error appears and you can retry.
  2. Add, remove, and reorder fields in the Build tab. The preview updates beside them.
  3. Open Settings to choose the completion message and CRM actions.
  4. Save the draft, then select Publish when it is ready to receive submissions.
  5. Use Share for the hosted link or embed code. Incoming answers appear under Submissions.

Available field types

Text and contact

Short text, long text, email address, phone number, and number.

Choices

Dropdown, radio group, and checkbox. Dropdown and radio accept one configured option.

Date and file

A date picker or one public file upload. Files can be images, PDF, text, CSV, Word, or DOCX up to 10 MB.

Structure and data

A heading separates sections, while a hidden field carries submission data without showing an input.

A form can contain up to 60 fields. Each field has a stable key used in submission data and automations, plus an optional placeholder, help text, required flag, and CRM mapping. Choice fields can contain up to 50 options.

Validation and contact mapping

Required fields are enforced on the server, not only in the browser. Systemly also checks email and phone formats, accepted choice values, dates, number limits, text lengths, and file URLs before storing a submission.

Use Maps to contact field for answers such as first name, last name, email, phone, business, address, location, or notes. Field keys are unique within the form and may contain lowercase letters, numbers, and underscores.

Choose what happens on submit

  • Show a custom success message or redirect the visitor to a valid web address.
  • Create a CRM contact from mapped email or phone data, assign its status and lead source, and apply selected tags. If the email or phone matches an existing contact, the form only fills that contact's empty fields and adds new custom fields. It never replaces a saved value, so a stranger cannot change a customer's details through a public form. The full answers are always kept under Submissions.
  • Create an opportunity in a selected pipeline with the form name in its title.
  • Notify up to 10 team addresses with an email containing the answers.
  • Start workflows listening for the Form submitted trigger.

Review submissions

The Submissions tab shows the newest responses, their submitted values, and a link to the CRM contact when one was matched or created. Hosted and embedded versions feed the same list and trigger the same follow-up behavior.

Public submissions are rate limited per form and visitor address to protect the workspace from automated abuse. File uploads have a separate, tighter rate limit.
